Lincoln Park Care Center is a highly regarded 24-hour skilled nursing facility known for outstanding patient care in an elegant therapeutic environment. We provide up to seven-day-a-week therapy based on an individual’s needs.

At Lincoln Park Care Center, our staff is dedicated to working with our residents to help them acquire and relearn skills that are needed to live an active and independent lifestyle. Our experienced Physical Therapists work with each resident to improve strength and restore function through the use of therapeutic exercises. At Lincoln Park, our Rehabilitation Therapists wants each of our residents to maximize his or her potential in a dignified and comfortable manner, so they therefore put special emphasis on “pain management”. Whether recuperating from an orthopedic procedure, a cardiac event, a CVA (stroke), or a lengthy hospitalization, our team of trained therapists will ensure that you reach your maximum level of independence.

At Lincoln Park Care Center, our goal is for our residents to return to an active and independent lifestyle, and so our Occupational Therapy program is aimed at improving fine motor, coordination, and cognitive skills and teaches functional and self-care skills commonly referred to as Activities of Daily Living (ADL). At Lincoln Park, our staff understand the importance of comprehensive care, which is why our trained Physical and Occupation Therapists work hand-in-hand with our trained Speech and Language Pathologists to help our residents communicate more effectively by using advanced techniques in oral and pharyngeal sensorimotor training

Our social work and case management teams provide continuous support to both residents and their families to ensure a safe and appropriate return to the community.
